In an effort to keep our teams and community safe, we are requiring all team members to show proof that they are fully vaccinated. At USHG this is defined as: 

  • Moderna and Pfizer vaccines: Your 2-shot vaccination series PLUS a booster 180 days (6 months) after completing your primary vaccination series.

  • J&J vaccine: plus a booster shot 60 days (2 months) after completing your primary vaccination.

Valid forms of vaccination status include a physical vaccination card or a state-provided digital record.
